Evaluation

This section outlines the steps for facilitators to record their teaching activities and submit evaluation data. Most of the evaluation tools are imbedded as part of learning activities to make them easy for participants and facilitators to use.

Facilitators may find the Checklist: CC&S Evaluation helpful to organize the required paperwork and evaluation data.

Every time a Cancer Clear & Simple educational series is planned and conducted, facilitators will:

1.  Register the series: Facilitators complete an on-line form recording the basics of when and where the series will be held.
2. Submit evaluation data: After registering your series, facilitators will be issued a unique link to submit evaluation data. These data include:
   • Series Description
   • Check Your Understanding (Pretest and Posttest)
   • Reflection Activity
   • SMART Goals Postcard Activity
3. Distribute and collect:
   • Participant Consent Form
   • Session Sign-in Sheet
